It might happen in other games, but Majora's Mask is the only one I've seen it in...
It only happens during scenes where they use blurring effects, which happens to be a lot in Majora's Mask. There's this strange zig-zag down the middle-left side of the display, with the left side blurred more than the right side. Lines also seem to not match up sometimes, as if the two sides are a bit out of sync.
Very noticeable at the startup:
