Not every OS that's not Windows is fully posix compiliant. I would suggest that it's only used on systems where it's sure that they're posix compilant, and that's mainly *BSD and Linux.
Also, there are some Win32 Compilers that don't define WIN32 but something else, like __WINDOWS__ for example.
