How do I load a rom or image? I select run but it doesn't let me pick a file just shows the starting menu thing and if I select play it says insert game disc.

Use the gdrImage GD-ROM plugin, and then go to Config --> GD-ROM to choose the disc image you want to use.

which part is the image? I downloaded rom but it only has 2 files a BIN and a CUE

BIN is the image, CUE is the description file that describes the image. Check the filesizes. having said that, you image is non-selfboot, forcing to use a bootdisc to get the game running. I don't have much faith in bootdiscs personally, but some of them work.

Does bootdisk mean you burn it onto a CD or what?

Like with most things related to Dreamcast emulation, you can just use a bootdisc image. If you want to burn the image to a disc and run it from there, I'm not stopping you though.
